Prior to each service commencing, carry out with customers (and/or their chosen representatives) an assessment of what the customer needs and what they would like to achieve from their care and support, together with a risk assessment
Create and agree a written individually tailored plan of care and support that respects the customer's wishes and promotes their dignity and privacy. Agree appropriate risk control measures to reduce identified risks. Keep care and support plans up to date Providing personal care duties
Provide the customer and, where appropriate their representatives, with information about the service so they are clear about what to expect and how they can raise any concerns.
Keep all information about customers and their families secure and confidential except where policy require you to share to protect the interests of our customers.
Carry out regular reviews with customers in line with the Bluebird Care Model.

Willing to seek guidance when needed and follow instructionsExcellent time keeper and reliableGood understanding of the needs of people who require care and support at home and the principles of good care practiceRelevant social care qualifications and willing to work towards further qualificationsUnderstanding of what confidentiality means in relation to homecare services and why this is importantGood understanding of health and safety matters in relation to homecare services and of safeguarding people at risk of harmExperience of communicating clearly and building positive working relationships with both staff and people who use social care services and social and health care professionalsExperience of care servicesAble to write clear risk assessments, care and support plans and other recordsYou must have:
Proven Care experience
Right to work in the UK
Excellent level of English both written and spoken
Full and valid UK driving licence
Access to a car
Good ICT skills.
