Our team is seeking a compassionate Hospice Clinical Supervisor to join our extraordinary care team. The role involves ensuring the best support for patients through team collaboration and leadership. The ideal candidate will have a Bachelor's degree, relevant licenses, and experience in Hospice care and supervision.
Schedule and Shift Details
? Monday-Friday, 8am-5pm 2-3 days in office, other days remote from home
Key Responsibilities
? Ensure productivity goals of clinical staff are met
? Ensure case management guidelines are being followed by each team member
? Assign patients to multidisciplinary team and adjust schedules as needed
? Serve as the first line for resolution of concerns and issues for the staff
? Train, develop, and mentor staff, including creating and recruiting, administering orientation process and patient assignment, providing employees with continuous training and education, and performing employee reviews, exits, counseling, and meetings
? Communicate regularly with support staff to assess current needs, determine level of accountability, assign patients, and provide supportive discussion related to areas of responsibility and job duties
? Communicate with leadership regarding continuous quality improvement, problem solving, and other issues related to the overall effectiveness of the Hospice program
? Develop staff to meet quality, safety, and competency requirements by assisting with training and professional development of new and current staff, annual joint visits, and setting up vacation coverage
Minimum Requirements
Required: 2+ years of Hospice experience and valid RN number
ANY of the following certifications/licenses:
American Heart Association Advanced Cardiac Life Support (AHA ACLS)American Heart Association Basic Life Support (AHA BLS)
