What you will do:
The role of the Senior Diabetes Educator in an outpatient setting is to lead/support improvement efforts that affect patients hospitalized with diabetes or hyperglycemia. This encompasses patient and family education, as well as education of interdisciplinary team members. The CDCES will also act as a lead/support to organizational achievement of diabetes-related quality metrics and performance outcomes. Diabetes self-management education and support is widely recognized as an important component of patient care, recovery, and reduced cost in the hospital setting. This position works collaboratively with interdisciplinary leadership and teams throughout the organization.
Responsibilities:
Diabetes Care Coordination: Collaborates with Endocrine team to develop system improvement plans as needed that incorporate staff education for all disciplines as they interact in caring for patients and families with diabetes and/or hyperglycemia.
Education: Provide input into patient self-management education planning at patient and institutional level.
Change Agent: Uses an evidence-based approach to facilitate the initiation of, adoption of, and adaptation to change.
Leader: Participates in organizational committees and/or shared governance councils.
Professional Role Competence and Growth: Maintains professional practice knowledge base needed to help nurses prepare for contemporary nursing practice.
What you will need:
BS required, MS preferred.
Must hold board certification as Certified Diabetes Care and Education Specialist (CDCES).
5 years' experience as practicing CDCES.
Experience in program development and previous leadership experience preferred.
Strong computer skills, competent in Word, Excel, and PowerPoint.
Strong communication skills.
The ability to analyze and interpret regulatory guidelines and mandates.
Previous experience with project or program development preferred.
Pay Range: $35.75- $47.78. Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
