.ANNEX 21 - Applicants who do not meet the essential criteria will be considered under the Annex 21 training programme, where whilst undergoing training they will be paid 75% of the top of Band 7 and on completion of training will then move on to Band 7.The Role University Hospital Monklands has two fantastic opportunities for Advanced Practitioners. You will work autonomously across Primary, Community and Acute Services during the in and out of hours period, covering seven days including public holidays. They will be responsible for the clinical decision making for the population including assessment and treatment of patients in across the Acute care setting.You will work closely with the multidisciplinary team (MDT), communicating on a daily basis about any patient they have seen who needs to be followed up by other services.You will be responsible for providing assessment, clinical care and management of both stable and acutely unwell patients and his/her family, making complex, autonomous decisions using advanced skills and knowledge, having overall responsibility for the assessment of care needs and the delivery of advanced care.
